This is the semiannual Post-Acute leadership educational conference. The conference is designed with long term care nurses in mind to disseminate the latest updates in AI, guardianships, falls, hospice, regulatory issues, and cybersecurity.

ANCC Learning Outcomes
At the conclusion of this activity, learners will be able to:

  1. Discuss fallprevention strategies across the care continuum, including acute care, SNF, and home care settings.
  2. Discuss the importance of transitional fall prevention techniques from inpatient through being back home to reduce fall risk
  3. Identify at least three ways clinical storytelling and objective quality data can improve digital trust and organizational reputation.
  4. Differentiate digital messaging strategies for adult children vs. techsavvy seniors seeking care.
  5. Distinguish among a guardian of the person, guardian of the estate, and emergency guardianship.
  6. Identify alternatives to guardianship and the least restrictive decision making supports available to patients.
  7. Describe the purpose of hospice care and its role in improving quality of life for patients with terminal diagnoses.
  8. Discuss clinical indicators that support appropriate hospice referral and patient family education.
  9. Explain the procedural steps involved in the appeal and recovery process following an IJ citation.
  10. Identify the roles and responsibilities of nursing and leadership teams during IJ mitigation.
  11. Describe common cybersecurity threats and their impact on healthcare operations and patient privacy.
  12. Apply HIPAA, HITECH, and organizational privacy policies to protect PHI in daily clinical practice.

Accreditation

Lehigh Valley Health Network is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation. Completion Criteria: Registered Nurses attending the entire program will receive 6 Contact Hour(s).

The purpose of this presentation is to educate the audience on available durable medical equipment to aid in fall prevention and recovery, review potential transitional fall prevention techniques that can span across the continuum, review the impact of basic activities of daily living on fall prevention, more specifically confusion and mobility, and discuss the skilled nursing facility perspective on fall prevention and regulatory compliance.
